Welcome to MCMH + Clinics Social Services
At MCMH + Clinics, we recognize our patients as a whole which involves physical, mental and psychosocial aspects. A compassionate environment is essential for our patients and families as we encourage them in maintaining their health and safety regarding after-care decisions from a hospital stay, with any outpatient encounter or call from the community.
We are available to assist you in specific areas including:
Transition planning of post-hospital services
- Home Health Care
- Durable Medical Equipment
- Nursing Home Care
- Meal Services
Attending to psychosocial needs
- Community based program referrals
- Financial Concerns
- Emotional Stress
Advance Directives
An Advance Directive is a general term for legal documents (such as a Medical Living Will and Durable Power of Attorney for Health Care) that state a person's wishes for life-sustaining medical treatments and naming someone to make healthcare decisions for you if you become unable to do so. Adults of every age should consider forming an Advance Directive. We encourage you to talk with your physician about this and involve family members in the discussion, too.
